The organisation that owns or occupies the facility holds the duty. Appointing an operator does not remove it. What an operator does is run the system, hold the competence and produce the evidence, so the duty holder can show the duty has been met.
Audit frequency depends on the size of the facility, whether it has a pool, and what the insurer and the duty holder require. Facilities with a pool sit at the frequent end, because plant and water safety carry the heaviest obligations. The cycle is agreed for your site before it starts.
